It is important to your progress as a student, and to the integrity of the UNC School of Education and the University as a whole, that clear goals and work products are established for your EDUC 994 course this semester. Therefore, please complete this form, sign it, and submit it electronically. This form must be submitted by 8 a.m. the day before classes start for the semester or term.

Further, I understand that the University of North Carolina at Chapel Hill does not allow a grade of incomplete for EDUC 994 credits.
